Hide the form from visitors who have submitted it already
You can configure your subscription form to be hidden from visitors that have already seen and submitted it.
To hide the form from visitors that have already submitted:

In the settings for your subscription form, in the left-hand navigation pane, click Set visibility settings.

Scroll to When should this form be displayed?, and select the Don't show the form to people who have already submitted it check box .

Click Next.
